We are eager to connect with you, and we are here to answer any questions you may have regarding your unique child and how we can best support them. Our goal is to provide you with all the information you need about how our program operates, empowering you to make the best decision for your child and family.

Step 1

Questionnaire & Phone Consultation

​Please fill out the screening questionnaire below to schedule your free phone consultation. During the call, we will review your responses on the questionnaire and gather any additional information you believe is important for us to know so that we can help support your child. This process allows us to determine if Patterns of Hope is the right fit for your child, and you will have the information you need about our program to make the best decision for your child and your family. Because each child is different and requires different support, the cost of our program also differs. We will provide payment information during this phone consultation.

 

Optional Mini-Assessment

If you are unable to complete the screening questionnaire, or prefer to meet in person, we are happy to accommodate you.

 

1-hour online or in-person session: $50.00

 

At this assessment, we will assess your child/children’s:

 

Gross muscle coordination and balance

 

Reflexes (just the main ones)

 

Tests for cerebellar involvement

 

Tests for patterns of motor development

 

 

Step 2

The Full Comprehensive Assessment is used to create a baseline of your child’s developmental functioning. Each child has a different pattern, and with this assessment, we will create an individual plan based on their individual needs. This assessment is going to be your biggest investment (of both time and money), but it is what allows us to determine the right program for your child/children.

 

4-hour session

 

The full assessment includes:

 

Tests for Gross Muscle Coordination and Balance

 

Tests for Patterns of Motor Development

 

Tests for Cerebellar Involvement

 

Tests for Dysdiadochokinesia (rapid alternate movements)

 

Tests for Aberrant Reflexes (both primitive and postural)

 

Oculo-Motor Screening

 

Visual-Perception Screening

 

Diagnostic Assessment Report

 

First Exercise

 

1-hour Phone Assessment Review to discuss the results of the assessment

 

Continuous Support and Weekly Check-ins

 

 

I never want finances to be the reason a child can’t get the help they need. I will do whatever I can to help make Patterns of Hope cost-effective for your family. Unfortunately, we do not accept insurance at this time. We do accept cash, check, Visa, MasterCard, American Express, and Discover Card for a small fee. I also take payments through Venmo. I am always happy to create a payment plan that works for your family.

Review Assessments

2-hour session

Follow-up visits are scheduled every 8 weeks. During these visits, we will re-screen and make adjustments to your child's exercise.

 

If you ever have any concerns about whether or not you are doing an exercise correctly, you can schedule a time to have it checked at no charge. Your child's safety and well-being are always my utmost concern.
